Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at EverHeart Hospice. As a LPN, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Key Responsibilities
  • Direct Patient Care: Provide competent nursing care to assigned patients through skilled symptom management, emotional support, and timely response to patient and family needs.
  • Routine Home Care Visits: Conduct end-of-life/continuous care visits, referrals, care center assistance, chart reviews, and other duties as assigned.
  • Plan of Care Implementation: Provide patient/family services according to established Plan of Care and signed physician orders.
  • Communication and Reporting: Recognize and report promptly to the registered nurse any changes in the mental and/or physical status of the patient, both written and oral when appropriate.
  • Education and Support: Explain hospice services and benefits in regards to Medicare, Medicaid, and Private Insurances, and knows when to use Palliative Care Interventions' and ABN's.
  • Ordering and Facilitating: Obtain and enter orders from Medical Directors and Physicians when deemed appropriate, order appropriate Durable Medical Equipment (DME) for patients, and facilitate continuity of care by coordinating admission of home-care patients to in-patient settings.
  • Flexibility and Adaptability: Work evenings and weekends as needed to meet the needs of patients and families, and maintain a backup schedule.
  • Documentation and Compliance: Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  • Clinical Competency: Maintain clinical competency for assigned responsibilities and procedures.
Requirements
  • Graduation from an approved School of Nursing.
  • Current LPN licensure in the State of Ohio and the eligibility to obtain licensure in the State of Indiana or current LPN licensure in the State of Indiana and the eligibility to obtain licensure in the State of Ohio.
  • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
  • Hospice experience preferred.
