Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Five Star Care (2). As a key member of our healthcare team, you will play a vital role in providing exceptional care to our residents.

Responsibilities:

  • Administer medications and treatments to residents, ensuring their safety and well-being.
  • Assist with the admission, transfer, and discharge of residents, maintaining accurate records and communicating effectively with the interdisciplinary team.
  • Document resident care services in a timely and accurate manner, adhering to facility policies and procedures.
  • Educate residents and their families on treatment plans, rehabilitation goals, and changes in their condition, promoting informed decision-making and empowerment.
  • Collaborate with physicians to develop and implement resident care plans, ensuring alignment with facility standards and DOH regulations.
  • Maintain resident confidentiality and privacy, adhering to HIPAA guidelines and facility policies.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
  • Work collaboratively with the interdisciplinary team to resolve resident concerns and address their needs in a timely and effective manner.

Requirements:

  • Graduate of an accredited LPN program or equivalent.
  • Experience in long-term care or a related field preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Positive attitude, strong work ethic, and a commitment to delivering exceptional care.

Shift Availability:

All shifts available, including days, evenings, and nights.
