Job Overview

Position: Registered Nurse, Patient Care Coordinator

Department: Medical-Surgical/Emergency Department

Work Schedule: 40 hours per week, including one weekend every six weeks

Role Summary:

The Patient Care Coordinator is responsible for overseeing the coordination of utilization reviews, discharge planning, skilled care management, and the SHIIP program. This role involves aligning patient care requirements with available resources, ensuring that patients receive appropriate levels of care, and making adjustments as necessary. The coordinator is accountable for managing care across inpatient, outpatient, and skilled services to achieve optimal clinical and patient care outcomes while utilizing all available resources effectively.

This position requires collaboration with healthcare providers, staff, and other professionals to deliver comprehensive care management and coordination throughout the healthcare continuum. The coordinator plays a crucial role in assisting patients and their families in navigating the healthcare system, enhancing the coordination of care, and improving the overall patient and family experience. Additionally, the coordinator addresses any concerns or special needs that may arise during the patient's treatment journey.

Qualifications:

  • Must hold a Registered Nurse license in the State of Iowa. A Bachelor's degree in nursing is preferred but not mandatory.
  • Experience in health insurance, case management, transitions of care, and information technology is essential.
  • Possess strong problem-solving abilities.
  • Ability to adapt to a flexible work schedule and manage frequent interruptions.
  • Capable of prioritizing tasks to meet deadlines for daily responsibilities and special projects.
  • Must be able to work independently with minimal supervision.
  • Effective verbal and written communication skills, along with excellent interpersonal abilities.
  • Must demonstrate a high level of responsibility regarding confidential matters.
