Job Summary

Zufall Health is seeking a highly skilled Prenatal Case Manager to join our team. As a key member of our Care Team, the Prenatal Case Manager will play a vital role in ensuring the health and well-being of our prenatal patients and their families.

Key Responsibilities
  • Work collaboratively with the Care Team to identify prenatal patients in need of services and communicate with providers to ensure seamless care coordination.
  • Meet with prenatal patients and their families to assess their needs and develop personalized plans to address barriers to healthy outcomes.
  • Document patient interactions and progress in our electronic medical record system, ensuring accurate and timely updates.
  • Follow up with patients who have missed appointments and reschedule as needed, ensuring timely access to care.
  • Assist patients with transportation, translation, and other barriers to attending appointments, promoting equity and access to care.
  • Facilitate referrals to community resources and services, including behavioral health and enabling services, to support patients' overall well-being.
  • Act as an advocate for patients and their families, navigating complex systems and ensuring their voices are heard.
  • Conduct educational sessions on prenatal care and related topics, promoting health literacy and empowerment.
Requirements
  • Minimum Bachelor's Degree in a health-related field or equivalent experience.
  • At least two years of experience in a clinical setting, with a focus on prenatal care and community health.
  • Experience working with medical insurance, Medicaid, HMO, and ACA, as well as PRAspect and GEMS Authorizations.
  • Knowledge of Microsoft Office, Excel, and electronic medical records, with a strong understanding of trauma-informed care principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
