We are seeking a skilled and compassionate Nurse Practitioner to join our team at Lancaster General Health. As a key member of our healthcare team, you will provide high-quality patient care and support to patients of all ages, from pediatric to geriatric.
Key Responsibilities- Provide clinical advice and guidance to patients via phone or electronic correspondence, utilizing clinical judgment and triage protocols.
- Deliver patient education on preventive care, chronic disease management, acute diagnoses, medications, and other relevant topics to promote optimal patient outcomes.
- Prepare patients for visits by utilizing standardized rooming procedures.
- Administer injections and medications as prescribed by providers, adhering to department protocols, regulatory requirements, and professional standards.
- Assist providers with various procedures, minor surgeries, and exams as designated.
- Perform Point of Care Testing, including hearing, flu, urine, and strep tests.
- Collect and prepare blood or other laboratory specimens for further testing.
- Authorize prescription refills as instructed by providers.
- Assist with test tracking and monitoring overdue results.
- Remove sutures and change dressings on wounds as necessary.
- Ensure exam rooms are adequately supplied and cleaned after each patient visit, maintaining and cleaning medical instruments and equipment.
- Collaborate with providers and patients to review and revise care plans, assessing patient progress and following up as needed.
- Participate in proactive outreach for patients requiring preventive and chronic disease services.
- Engage in care coordination activities among specialists and transitions of care to obtain necessary patient information.
- Assist with referrals, education, and community resources.
- Contribute to performance improvement initiatives to enhance patient experience and quality of care.
- Participate in daily huddles with providers.
- Act as a patient experience advocate, promoting a positive influence when interacting with patients, visitors, and colleagues.
- Maintain confidentiality of patient information according to Lancaster General Health's privacy and confidentiality policies and procedures.
- Completion of a LPN or RN educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing.
- AHA CPR/BLS certification required within 30 days of hire.
- One to two years of nursing experience in a primary or specialty care physician's office.
- Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience.
