Job Title: Nurse Practitioner

Center Point DAAC is seeking a skilled and compassionate Nurse Practitioner to provide high-quality patient care, conduct intake and annual physicals, health screenings, and consult with clients regarding dosing issues.

Key Responsibilities:
  • Coordinate with Medical Director on intake and initial physician orders.
  • Perform Medical History and Physical Examinations on all REAP clients at intake.
  • Participate in necessary blood drawings, prepare blood specimens for laboratory, and ensure pick-up. Dosing duties may include preparing, dispensing, and cleaning up after daily medication dispensing and evaluating side effects and patient reactions to medication daily.
  • Administer PPDs (TB tests) and follow up with Public Health for patient treatment when test results are positive.
  • Perform Health Screenings and PPDs for new hires and annually for CP-DAAC staff.
  • Prescribe psychiatric and medication-assisted treatment medications, electronically send them to pharmacies, and follow up with any pharmacy inquiries.
  • Responsible for ordering all medical and examination supplies.
  • Perform in-person Mental Health assessments, crisis interventions, and coordinate with Medical Director on mental health clients.
  • Ensure correspondence and lab reports are signed by Medical Director and filed in patient charts. Perform annual History and Physical Reports on appropriate clients.
  • Follow up and provide recommendations and referrals on identified medical problems or positive findings from History and Physical reports and other health-related client visits.
  • Serve as a liaison with doctors, hospitals, mental health care workers, and other professionals regarding patients.
  • Ensure compliance with Title IX Regulations and Federal Law regarding intake requirements. Demonstrate positional knowledge of SMARTCARE database and other computer applications as required.
  • Participate in Medical and Psychiatric Program Development across all of CP DAAC's programs.
  • Direct liaison to Medical Director.
  • Provide training, support, and supervision for dispensing staff.
  • Coordinate with Sonoma County Detention Facilities to administer medication to incarcerated patients. Coordinate with hospitals and inpatient programs regarding patient medication.
  • Perform Impaired Client Assessments for clients appearing under the influence.
  • Participate in updating the Dispensing Manual and REAP UOM.
Requirements:
  • Current license as a Nurse Practitioner with psychiatric training and experience.
  • Master's level education or higher.
  • Education and/or experience demonstrating basic counseling skills and knowledge of addiction and treatment.
  • Possess good written and verbal communication skills.
  • Accurate mathematical calculation skills.
  • Ability to maintain productive work relationships with patients, employees, management, external agencies, and the public.
  • Ability to maintain appropriate boundaries with clients and staff as defined by DAAC's Code of Conduct.
  • Ability to use effective judgment in therapeutic and crisis situations.
  • Ability to recognize and manage personal bias.
  • Ability to solve problems and resolve conflicts effectively.
  • Ability to maintain own psychological well-being.
  • Maintains current CPR and First Aid certification.
  • Maintains current license/certification and supporting CEUs as required.
  • Possesses a valid California Driver's License.
  • Unencumbered California State License for necessary functions.
