About the Role

We are seeking a highly skilled and experienced Field Nurse Case Manager to join our team at Paradigm. As a Field Nurse Case Manager, you will be responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.

Key Responsibilities
  • Assess and evaluate injured/disabled individuals to determine the extent of their injuries and develop a plan for their care and rehabilitation.
  • Coordinate with insurance carriers, medical care providers, attorneys, employers, and employees to ensure that the injured worker receives the necessary medical treatment and support.
  • Monitor the progress of the injured worker and report results back to the employer and insurance carriers.
  • Develop and implement case management plans to ensure that the injured worker receives the necessary medical treatment and support.
  • Collaborate with other healthcare professionals to ensure that the injured worker receives comprehensive and coordinated care.
Requirements
  • Current Registered Nurse (RN) license in GA
  • CCM, CRRN, or COHN required
  • GA Rehabilitation Supplier required
  • Experience in workers compensation case management, preferred
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
  • Valid Driver's License, and good driving history
