Job Title: Healthcare Professional for Patient Care

About the Opportunity

The successful candidate will be responsible for providing quality patient care and performing technical skilled care in the patient's home. This role requires strong communication and interpersonal skills, as well as the ability to work independently and collaboratively as part of a healthcare team.

Key Responsibilities

  1. Provide quality patient care and perform technical skilled care in the patient's home
  2. Instruct patients' families in the care of the patient and maintenance of a healthy environment
  3. Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily

Requirements

  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • CPR Certification required
