Patient Care Navigator

Overview:
The Registered Nurse plays a crucial role in patient assessment and education, collaborating with physicians during consultations and managing follow-ups on prescriptions, tests, and procedures. It is essential to maintain accurate documentation to ensure comprehensive records of patient visits.
Key Responsibilities:
- Conducts initial assessments of patient needs and formulates treatment plans by measuring vital signs, collecting patient history, and identifying primary concerns. Provides physicians with evaluations of patient conditions.
- Administers tests as prescribed by the physician.
- Prepares medical equipment and supports physicians during examinations and treatments.
- Collects laboratory specimens and organizes them for transport, ensuring all necessary documentation is completed.
- Dispenses prescribed oral medications and administers injections as required.
- Facilitates patient education and provides instructional support.
- Evaluates patients to identify emergencies and delivers immediate assistance when necessary, alerting the healthcare provider of critical situations.
- Contributes to the creation and documentation of patient care guidelines and educational resources.
- Analyzes various medical, statistical, and operational data to prepare reports for measuring outcomes and program effectiveness.
- Reviews treatment documentation to ensure that all necessary information for reimbursement is captured and directs relevant details to the appropriate departments.
- Ensures that all patient information is accurately recorded in the Electronic Health Record system.
Qualifications:
High school diploma or GED along with completion of an accredited nursing program.
Licensure: Current RN license in the state and valid CPR certification.
Experience: A minimum of two years of clinical experience in a physician's office setting is required.
