We are seeking a skilled Registered Nurse to join our team as a Home Health Infusion RN. In this role, you will have the opportunity to work with patients who require infusion therapy in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive patient assessments to identify individual needs and develop personalized care plans
- Administer infusion therapy, including pump refills and dose adjustments, as prescribed by physicians
- Provide education and support to patients and caregivers on infusion therapy and related health topics
- Collaborate with healthcare teams to ensure seamless care coordination and communication
- Document patient information and treatment plans accurately and efficiently
- Current licensure as a Registered Nurse in the state of Illinois
- 3+ years of experience in infusion nursing or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Intermediate computer skills with Microsoft Office Suite and electronic medical records
