Position Summary / Career Interest:
The Clinical Pharmacy Specialist is responsible for ensuring the safe and effective administration of medications by verifying medication orders and assessing optimal drug selection, dosage regimens, and monitoring drug therapy outcomes. This role involves collaboration with healthcare professionals to identify alternative therapies when necessary, ensuring the highest standard of patient care.

Key Responsibilities and Essential Functions
  • Engages in performance improvement initiatives within the organization and department, contributing innovative ideas to enhance pharmaceutical care across clinical settings and the pharmacy department.
  • Actively participates in multidisciplinary teams by attending patient care rounds and delivering patient-centered pharmaceutical care through thorough assessment and monitoring of drug therapies to ensure safe and cost-effective treatment plans.
  • Supports departmental staffing requirements or alternative scheduling to assist with coverage of additional shifts as needed.
  • Ensures prompt processing of medication orders by participating in order entry, verification, and timely resolution of any issues with medication orders.
  • Conducts comprehensive reviews of patient-specific medication profiles, implementing protocol-driven drug therapies, including pharmacokinetic dosing adjustments and IV to PO conversions, with appropriate documentation. Adjusts dosages based on individual patient parameters such as renal or hepatic function.
  • Regularly reports medication errors and adverse drug reactions, participating in reviews and activities aimed at establishing safe medication practices.
  • Involves in medication reconciliation during patient admissions, transfers, and discharges, providing education to patients as necessary.
  • Shares expertise in pharmacotherapy by coordinating and delivering educational sessions for both pharmacy and non-pharmacy staff.
  • Must possess the professional, clinical, and technical competencies required for the assigned department or unit.

Required Education and Experience
  • Bachelor's Degree in Pharmacy
Preferred Education and Experience
  • Pharmacy Doctorate
  • Completion of an accredited postgraduate training program (PGY-1 and/or PGY-2) residency program
Required Licensure and Certification
  • Licensed Pharmacist - State Board of Pharmacy
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Licensure and Certification
  • Pediatric Advanced Life Support (PALS) - American Heart Association (AHA)
  • Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA)
  • Neonatal Resuscitation Program (NRP) - American Academy of Pediatrics (AAP)
