This is a TDY grant funded position with the Yukon Kuskokwim Health Corporation. The Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting.
Key Responsibilities- Diabetes Education and Care: Provide comprehensive diabetes education and care for patients via telephonic communication, video teleconferencing, and in-person education sessions.
- Health Promotion Activities: Assist in the planning, coordinating, and providing of diabetes-related health promotion activities.
- Collaboration and Communication: Work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care.
- Registry Maintenance: Assist in the maintenance of the Diabetes Registry and grant requirements.
- Outreach and Community Engagement: Participate in all program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events.
- Department Planning and Goal Setting: Participate in department planning and goal setting to achieve excellence in patient care and partnership with our tribal leaders.
- Education: Must have completed a program of education in nursing from an NLN approved institution. BSN required, MSN preferred.
- Experience: Two (2) years experience in a clinical health setting.
- Collaboration and Project Completion: Demonstrated experience collaborating with a variety of disciplines for project completion.
- Diabetes Education Background: Background in Diabetes Education required.
- Work Setting: Work in a typical office, hospital, and village clinic setting.
- Travel Requirements: This position requires occasional long working hours and travel to villages via small plane/sub-arctic conditions, moderate lifting of equipment.
- COVID-19 Vaccination: ALL TRAVELERS ARE REQUIRED TO SHOW PROOF OF COMPLETED COVID VACCINATIONS.
