Job Title: Clinic RN

We are seeking a skilled and compassionate Registered Nurse to join our team at Delta Health. As a Clinic RN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Clinic RN will be responsible for providing direct patient care, conducting health assessments, developing care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes. This is an excellent opportunity for a dedicated and detail-oriented nurse to make a meaningful difference in the lives of our patients.

Responsibilities:
  • Observe, interview, and assess patients to identify care needs
  • Provide direct care by performing comprehensive health assessments, developing differential diagnoses, and conducting specialized tests
  • Develop or assist others in development of care and treatment plans
  • Coordinate or conduct educational programs or in-service training sessions on topics such as clinical procedures
  • Collaborate with other healthcare professionals and service providers to ensure optimal patient care
  • Make clinical recommendations to physicians, other healthcare providers, insurance companies, patients, or healthcare organizations
  • Instruct nursing staff in areas such as the assessment, development, implementation, and evaluation of disability, illness, management, technology, or resources
  • Population Health management
  • Coumadin management
  • Other duties as assigned
Administrative Duties:
  • Builds strong positive communication with Providers and works as a team to enhance the success of the clinic
  • Develops strong inter-department teamwork to meet the needs of the patients and physicians and drive decision-making and other actions
  • Identified and implements opportunities to build employee morale and individual motivation
  • Analyzes and staffs back office staff to meet needs of healthcare provider
  • Works across organization in assisting with area of expertise; i.e. nursing, employee health, medical records, quality assurance
  • Plans, evaluates, recommends, and implements new initiatives when appropriate
  • Monitors, coaches, develops, and evaluates performance of staff on an ongoing basis in accordance with applicable performance standards
  • Establishes and maintains effective working relationships with vendors, employees, other managers, and Providers
  • Coordinates cleanliness, orderliness, and safety of working environment
  • Maintains complete confidentiality regarding medical records and personal information
Requirements:
  • Must be currently licensed to practice as a RN in the State of Colorado
  • Prefer bi-lingual in English and Spanish
  • Minimum or two (2) years experience in critical care nursing preferred
  • BLS required upon hire
