We offer flexible scheduling options to accommodate your needs, including full-time, part-time, and PRN positions with varied shift schedules – days, nights, and weekends.
Job Summary
This position involves providing direct patient care as a Licensed Practical Nurse (LPN/LVN) in a home setting, working collaboratively with our multidisciplinary team to deliver high-quality care and achieve outstanding clinical outcomes.
Responsibilities
* Provide skilled nursing care to patients in their homes, adhering to established policies and procedures
* Conduct thorough assessments of patient needs and develop individualized care plans
* Administer medications and treatments as ordered by physicians and collaborate with other healthcare professionals to ensure comprehensive care
* Monitor patient progress and report any concerns or changes to the healthcare team
* Maintain accurate records and documentation of patient interactions and care provided
Requirements
* Must be a licensed LPN/LVN in good standing in the state of employment
* Compact licenses must be transferred to the state of residence within 90 days if applicable
* Current CPR certification with hands-on component – we can assist with obtaining this requirement if necessary
* TB skin test result current within the last 12 months
* Six months prior hands-on nursing experience preferred but not required
* Reliable transportation for travel to patient locations
