Community Services Liaison

As a Client Services Coordinator at Standards Based Solutions Consulting, you will play a vital role in delivering high-quality services to our clients. You will be responsible for building and maintaining relationships with clients, families, and stakeholders, and ensuring that our Community Based Services (CBS) program runs smoothly.
Key Responsibilities:- Maintain accurate and timely weekly documents to inform service planning and delivery.
- Facilitate the intake and assessment process to ensure that clients receive comprehensive support.
- Conduct thorough assessments as necessary, utilizing evidence-based practices and adhering to established goals and objectives.
- Ensure service notes are clear, concise, and billable, promoting efficient documentation and reimbursement.
- Initiate and maintain client records, providing a centralized source of information for service providers.
- Participate actively in the documentation review process to guarantee accuracy and adherence to standards.
- Attend treatment team meetings to collaborate with colleagues and ensure effective service coordination.
- Assist with facilitating monthly supervisions and training sessions to enhance staff knowledge and skills.
- Age 18 or older.
- Basic knowledge of managed care billing principles.
- Exceptional written and verbal communication skills.
- Strong organizational abilities.
- A keen eye for detail.
We offer a competitive salary range of $35,360 - $37,440 annually, along with comprehensive benefits including Health, Dental, and Life Insurance. As a valued member of our team, you will also have access to opportunities for growth and development.
Requirements:- Bachelor's Degree in Human Services, Social Work, Psychology, or Sociology.
- Two years of experience working in the Human Services field or an internship/practicum (Preferred).
- Two years of experience as a Community Based Services Professional (Preferred).
- Three years as a Community Based Services Paraprofessional (Preferred).
- Qualified Professional (QP) or Qualified Mental Health Professional (QMHP) status (Preferred).
Estimated Annual Salary: $36,800.
