Community-Based Support Specialist

We are excited to offer a Community-Based Support Specialist position at Grady Health System. In this role, you will play a vital part in providing high-quality care and support to individuals with serious mental illness in their communities.

Key Duties:
- Collaborate with healthcare teams to develop personalized care plans for clients.
- Deliver group and individual training sessions on essential life skills and crisis management techniques.
- Establish strong relationships with clients, families, and community stakeholders to ensure seamless care transitions.
Essential Qualifications:
- Bachelor's degree in Behavioral Sciences or a related field.
- A minimum of three years of experience working with vulnerable populations, preferably in community-based settings.
- Basic Life Support (BLS) Certification or within 30 days of hire date.
- Preferred certifications include CTRS, CRC, or CPRP.
Compensation Package: Salary: $48,000 - $58,000 per year, plus mileage reimbursement and comprehensive benefits.
