Eliot's behavioral health program is seeking a highly skilled and compassionate Clinical Leader to oversee the coordination of medical, behavioral health, and Social Determinants of Health (SDOH) needs. The ideal candidate will have a strong commitment to mental health recovery, substance abuse disorders, and integrated care.
Key Responsibilities- Oversee the clinical operations of the Health Home Outreach Care Team.
- Provide supervision and clinical guidance on Annual MDS Comprehensive Assessment, Person-Centered Care Plan (PCCP), and Member Engagement process.
- Ensure coordination of services, assessments, and treatment planning to empower enrollees to exercise autonomy in all aspects of their life.
- Provide overall clinical supervision and oversight of care coordination and service delivery of assigned Health Home Team(s).
- Teach and model clinical interventions, strength-based strategies, and ensure services are culturally competent.
- Clinical licensure required.
- At least four years of experience working with people with severe and persistent mental illnesses, substance abuse disorders, or those with complex medical conditions.
- At least two years of supervisory experience.
- May require additional or specialized training with specialized populations and working knowledge of Evidence-Based Practices.
