We are seeking a skilled Patient Advocate - Substance Abuse Specialist to join our team at Spectrum Health Systems, Inc. in Westborough, MA.

About the Role:

  • As a Patient Advocate - Substance Abuse Specialist, you will provide documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems.
  • You will develop, implement, and revise individual treatment plans for assigned clients within established time frames with their participation.
  • Additionally, you will refer clients to appropriate community services as necessary and participate in multidisciplinary team treatment planning, case review, and case conferences.

Key Responsibilities:

  1. Deliver high-quality clinical services, including individual and group sessions, to support clients' recovery journey.
  2. Develop and maintain comprehensive treatment plans that address clients' unique needs and goals.
  3. Foster collaborative relationships with healthcare professionals, social workers, and other stakeholders to ensure seamless care coordination.

Requirements:

  • A minimum of a high school diploma or equivalent is required.
  • At least one year of experience working in a substance abuse treatment, case management role, or closely related field is preferred.
  • LADC I or CADAC certification is also highly desirable.
