About the Role:As a Home Health Physical Therapist, you will use your clinical skills and expertise to provide high-quality care to patients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Conduct thorough assessments of patients' physical abilities and develop personalized care plans to improve their mobility, strength, and overall well-being.
- Provide one-on-one physical therapy sessions to patients in their homes, using evidence-based techniques and interventions.
- Collaborate with other healthcare professionals, including nurses, occupational therapists, and speech therapists, to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and caregivers to educate them on the importance of physical therapy and provide guidance on how to support their loved ones.
- Document patient progress and outcomes, and participate in regular team meetings to discuss patient care and address any concerns.
- Active and current license to practice as a professional Physical Therapist (PT) in the state of PA.
- Previous home healthcare experience welcomed, but not required. We will train you.
- Ability to travel to cases as assigned.
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
- Flexible schedules for work/life balance: full-time, part-time, and per-diem. Scheduling is primarily during the daytime hours.
- 1:1 hands-on training supplemented with virtual learning.
- Ability to perform 1:1 direct client care and advocate for clients' needs.
- 24/7 clinical support as part of a larger care team with the highest safety standards.
