Job Title: Behavioral Health Counselor

Community Hope, Inc. is seeking a qualified Behavioral Health Counselor to join our team. As a Behavioral Health Counselor, you will provide direct adult rehabilitative support services to consumers in a community-based transitional housing residence.

Job Summary:

The Behavioral Health Counselor will provide on-site consumer supervision, participate as a team member in the maintenance and care of agency facilities, and ensure the safety of consumers in those facilities. The successful candidate will also transport consumers in agency vehicles, provide medication education, and supervise and assist consumers with rehabilitative services and daily living skills.

Responsibilities:
  • Provide direct adult rehabilitative support services to consumers in a community-based transitional housing residence.
  • Provide on-site consumer supervision and participate as a team member in the maintenance and care of agency facilities.
  • Ensure the safety of consumers in agency facilities.
  • Transport consumers in agency vehicles in a safe, cautious, and responsible manner.
  • Provide medication education and supervise and assist consumers with rehabilitative services and daily living skills.
  • Develop, supervise, and facilitate recreational and socialization activities for consumers.
  • Coordinate as directed any contacts with adjunctive services.
  • Document service delivery and maintain accurate and timely clinical documentation.
Qualifications:

The ideal candidate will possess a Bachelor's degree in Mental Health or a related discipline, or a Licensed R.N., or two years of college plus two years of related work or life experience, or licensed as a Practical Nurse (LPN) plus two years of related mental health experience, or a High School diploma and four years of related mental health experience.

The candidate must also possess a valid driver's license with a good driving record and be bilingual (English/Spanish) preferred but not required.
