Job Summary

We are seeking a skilled Registered Nurse Case Manager to join our team at the University of New Mexico. As a key member of our healthcare team, you will be responsible for coordinating patient care, ensuring quality and cost-effective outcomes, and collaborating with physicians, nurses, and other healthcare professionals to achieve optimal patient results.

Key Responsibilities
  • Coordinate patient care, including developing and implementing individualized treatment plans, conducting assessments, and monitoring patient progress.
  • Collaborate with healthcare teams to ensure seamless communication and coordination of patient care.
  • Manage patient safety, including identifying and reporting potential or actual safety concerns, and implementing strategies to prevent errors and adverse events.
  • Develop and maintain accurate and comprehensive patient records, including documentation of patient care, treatment plans, and outcomes.
  • Participate in quality improvement initiatives, including evaluating patient care systems, identifying areas for improvement, and implementing changes to enhance patient outcomes.
  • Provide education and support to patients and families, including teaching patients and families about their conditions, treatment options, and self-care strategies.
  • Collaborate with interdisciplinary teams to develop and implement care plans, including coordinating referrals, ordering tests and treatments, and communicating with patients and families.
Requirements
  • Current RN licensure in New Mexico or eligibility for licensure by reciprocal agreement.
  • 2 years of direct experience in a healthcare setting, preferably in case management or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment, with a high level of accuracy and attention to detail.
  • Basic Life Support (BLS) certification or equivalent.
Working Conditions

This position requires the ability to work in a fast-paced environment, with frequent interruptions and a high level of accuracy and attention to detail. The physical demands of this position include standing, walking, and sitting for extended periods, with occasional lifting of up to 20 pounds.

This is a light work position, with minor hazards including physical risks, dirt, dust, fumes, and noise. The working conditions are typical of a healthcare setting, with a focus on patient safety and quality care.
