Job Summary

We are seeking a skilled Case Manager to join our team at Texoma Community Center. As a Case Manager, you will provide rehabilitation services and support to individuals with persistent and severe mental illness, promoting their recovery and well-being.

Key Responsibilities
  • Provide case management services, including advocacy, crisis intervention, and skills training, in accordance with the Texas Administration Code and current state guidelines.
  • Operate under trauma-informed and recovery-oriented models of care, aligning with CCBHC standards.
  • Develop and implement individualized treatment plans, ensuring the needs of each client are met.
  • Collaborate with community stakeholders, including healthcare providers, social services, and family members, to ensure comprehensive support.
  • Maintain accurate records and reports, adhering to confidentiality and data protection policies.
Requirements
  • Bachelor's Degree in Social Work, Sociology, Psychology, or Counseling, with two years of Human Services experience.
  • Current driver's license and vehicle for use at work.
  • Ability to speak a second language relevant to the population served (preferred).
  • Self-motivated, organized, and detail-oriented, with excellent communication and interpersonal skills.
  • Knowledge of Microsoft Office Suite and ability to learn new software applications.
