We are seeking a skilled and compassionate Case Management Advocate to join our team at Special Service for Groups, Inc. - HOPICS. As a key member of our staff, you will be responsible for providing case management services to individuals and families experiencing homelessness.

This role is ideal for an individual with a passion for working with vulnerable populations and a strong commitment to providing exceptional customer service. You will work closely with clients, community partners, and internal teams to ensure seamless delivery of services and support.

Key Responsibilities

">
  • Provide case management services to clients, including assessing needs, developing service plans, and connecting them with community resources.
  • Collaborate with internal teams to ensure timely and effective delivery of services.
  • Develop and maintain relationships with community partners, including shelters, housing providers, and social service agencies.
  • Participate in weekly/monthly case conference meetings to discuss client progress and address any challenges.
  • Ensure accurate and timely data entry into HMIS and other relevant systems.
  • Conduct regular outreach and engagement activities to build trust and foster positive relationships with clients.
  • Assist clients with identifying and accessing permanent housing options, including rental assistance programs and supportive services.
  • Prepare and submit reports as required, including monthly summary reports and productivity metrics.

Requirements

To be successful in this role, you will need:

  • An associate degree in social services or a related field, with a minimum of 1-2 years of experience working with homeless individuals and families.
  • Strong case management skills, with experience working with diverse client populations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, community partners, and internal teams.
  • A valid driver's license and reliable transportation, with the ability to travel to various locations throughout the city.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and customer service.
  • Familiarity with Microsoft Office, including Word, Excel, and Outlook.

We offer a competitive salary range of $55,000 - $65,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision insurance, retirement savings plan, and paid time off.
