Job Summary:

We are seeking a compassionate and dedicated Home Health Aide to join our team at Modivcare. As a Home Health Aide, you will have the opportunity to make a meaningful difference in the lives of our clients, providing personalized care and support in the comfort of their own homes.

Responsibilities:

  • Provide Personal Care: Assist clients with activities of daily living, including meal preparation, light housekeeping, dressing, mobility, and personal hygiene.
  • Assist with Medication Reminders: Help clients manage their medication schedules and provide reminders as needed.
  • Provide Safety and Oversight: Ensure clients' safety and well-being by providing supervision and support as needed.
  • Communicate with Families and Healthcare Professionals: Maintain open communication with clients' families and healthcare professionals regarding their physical and emotional condition.
  • Document and Track Job Duties: Document and track job duties via electronic visit verification.

Requirements:

  • Certified Home Health Aide (CHHA) Certification: Possess a valid CHHA certification.
  • High School Diploma or Equivalent: Hold a high school diploma or equivalent.
  • Ability to Work Independently: Demonstrate the ability to work independently and make decisions in a fast-paced environment.
  • Excellent Communication Skills: Possess excellent communication skills to effectively interact with clients, families, and healthcare professionals.
  • Ability to Lift and Move Clients: Demonstrate the ability to lift and move clients as needed.
