Responsibilities:
  • Conduct home visits to assess and treat clients with physical therapy needs
  • Develop and implement individualized care plans to improve clients' mobility, strength, and function
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Provide education and support to clients and their families to promote independence and safety
  • Document client progress and communicate with the healthcare team
Requirements:
  • Current licensure as a Physical Therapist in Pennsylvania
  • Minimum of one year of experience as a Physical Therapist
  • Graduation from an accredited Physical Therapy program
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
