Key Responsibilities:

  • Provide compassionate care to clients and patients according to tailored Care Plans
  • Monitor and communicate changes in client/patient conditions to appropriate personnel
  • Keep families informed about care plans, health updates, and any concerns

Qualifications:

  • Current LPN or RN license in the state of practice
  • CPR certification and a clear TB test or chest x-ray
  • Preferred minimum of one year experience in home health, primary care, or healthcare settings
  • Valid driver's license and automobile insurance
  • Ability to perform various physical tasks and assist clients with mobility needs
