As an Intensive Support Coordinator, you will play a vital role in supporting individuals with intellectual and developmental disabilities to live independently and reach their full potential.

Key Responsibilities
  • Conduct thorough assessments and evaluations to identify the needs of individuals served
  • Develop and implement personalized care plans to support individuals in achieving their goals
  • Provide resources and support to individuals, their families, and guardians to promote independence and community involvement
  • Collaborate with interdisciplinary teams to ensure seamless service delivery
  • Maintain accurate and up-to-date records of individual progress and service delivery
Requirements
  • Bachelor's degree in Human Services, Social Work, Sociology, or Psychology
  • Valid driver's license and reliable transportation
  • Minimum of 2 years of experience in a similar role
  • Ability to travel up to 75% of the time
