Medical Case Manager

We are seeking a skilled Medical Case Manager to join our team at Overlake Medical Center. As a Medical Case Manager, you will be responsible for coordinating patient care and advocating for our patients' needs.
The ideal candidate will have a Master's degree in social work or a related field and possess one of the following licenses from the state of Washington: Licensed Independent Clinical Social Worker (LICSW), Licensed Advanced Social Worker (LASW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT). A minimum of two years of experience is required.
Key Responsibilities:
- Develop and implement individualized care plans that address patients' physical, emotional, and social needs.
- Collaborate with healthcare providers to ensure seamless care coordination.
- Foster strong relationships with community resources to provide patients with optimal support.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
