We are seeking a compassionate and highly skilled Medical Care Specialist to join our team at Abode Care Partners. As an Advanced Practice Nurse, you will provide evidence-based medical care to seniors and patients with complex needs in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.

About the Role

This is a unique opportunity for a dedicated nurse practitioner to make a meaningful difference in the lives of our patients while growing their medical skills and career. You will work collaboratively with healthcare professionals to develop and implement comprehensive plans of care, ensuring that each patient receives personalized attention and support.

As a Medical Care Specialist, you will be responsible for:

  • Conducting assessments, treatments, and interventions to address chronic and urgent episodic illnesses, minor injuries, accidents, and other health concerns
  • Ordering and interpreting laboratory and diagnostic testing as needed
  • Collaborating with caregivers and other healthcare professionals to ensure seamless interdisciplinary care
  • Providing referrals to other healthcare practitioners when necessary
  • Maintaining accurate and up-to-date patient medical records
Requirements

To be successful in this role, you will need:

  • An active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • A Master's degree in Nursing (MSN)
  • At least two years of nurse practitioner experience, with a focus on providing high-quality, patient-centered care
  • Excellent communication and interpersonal skills, with the ability to engage with patients, families, and healthcare teams
  • Strong time management and organizational skills, with a focus on prioritizing tasks and meeting deadlines
Compensation and Benefits

As a valued member of our team, you can expect:

  • A competitive salary range of $90,000 to $108,000 per year, based on experience

The estimated salary for this position is $100,500, based on industry standards and location. We offer a competitive compensation package, including a base salary, bonuses, and benefits, to attract and retain top talent in the field.
