We are seeking a skilled Family Medicine Physician to join our team at CenterWell Senior Primary Care in Fort Worth, Texas. As a Primary Care Physician, you will work closely with our experienced team of Advanced Practitioners and Care Coordinators to provide high-quality patient care.
Responsibilities:- Provide primary care services to patients in a team-based care environment
- Manage a panel of patients with a focus on coordinated care and value-based relationships
- Collaborate with our Care Team to address patient needs and social determinants of health
- Participate in quality improvement initiatives and stay up-to-date with best practices in geriatric and primary care medicine
- Doctor of Medicine or Osteopathic Medicine degree
- Board certification or eligibility in Geriatric Medicine, Family Medicine, or Internal Medicine
- Valid, unrestricted license in Texas
- Current, unrestricted DEA license
- Guaranteed base salary + bi-annual bonus
- Excellent benefit package, including health insurance, CME allowance, and malpractice insurance
- Relocation and sign-on bonus options
- 401(k) with employer match
- Life insurance and disability benefits
