Job Opportunity

Hueman Risk Adjustment is seeking a skilled Nurse Practitioner to join our team in completing in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in the Brownstown, IN area.

This unique opportunity offers a flexible schedule, allowing you to balance your professional goals with your personal life. As a Nurse Practitioner, you will have the chance to interact with patients in a more intimate setting, providing comprehensive, non-invasive physical assessments and reviewing medical history to ensure accurate and timely documentation.

Key Responsibilities:

  • Travel to and from patients' homes to conduct HRAs
  • Perform comprehensive physical assessments and review medical history
  • Ensure accurate and timely documentation of patient information
  • Provide basic patient education on wellness and preventative healthcare measures

Requirements:

  • Master's Degree or commensurate experience and satisfactory completion of NP license
  • Active State NP license
  • Active National board certification
  • Current BLS certification
  • 12 months of experience as a paid advanced clinical provider is preferred but not required
