Job Summary

We are seeking an experienced Registered Nurse (RN) for a part-time weekend position in our home healthcare team. The successful candidate will provide skilled nursing care, case management, and follow-up visits for patients in a home health setting.

The ideal candidate will have strong organizational and communication skills, as well as experience in med-surg, ICU, ER, acute settings, and preferably in home health. A valid driver's license, auto insurance, and reliable transportation are also required.

Key Responsibilities

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians in developing the initial plan of treatment based on physician orders and initial patient assessment.
  • Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.

Requirements

  • Diploma, Associate, or Bachelor Degree in Nursing.
  • Minimum of one year nursing experience preferred.
  • Strong med-surg, ICU, ER, acute experience.
  • Home health experience a plus.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
