Prologue Inc is seeking a highly motivated and compassionate Residential Care Specialist to join our team. As a Residential Care Specialist, you will play a vital role in providing direct psychiatric rehabilitation services and skills training to individuals recovering from mental illness.
Key Responsibilities:
- Implement individual service plans by providing direct support to residents in their homes, assisting them in developing the skills necessary for independence and success.
- Work collaboratively with residents, families, and other healthcare professionals to ensure a comprehensive and coordinated approach to care.
- Provide transportation to assist residents in accessing community resources and services.
- Document activities and services in log books, service tickets, incident reports, and progress notes.
- Assist residents with independent living skills, such as medication management, personal care, and household management.
- Communicate effectively with supervisors, colleagues, and other stakeholders to ensure a seamless continuum of care.
- Participate in crisis management and provide support to residents during times of crisis.
- Perform medication monitoring and ensure that residents' medication needs are met.
- Engage in activities that promote resident engagement, socialization, and community integration.
- Collaborate with the multidisciplinary team to develop and implement individualized service plans.
- Perform other duties as assigned by supervisors and/or management.
Requirements:
- A high school diploma or equivalent is required; experience in human service delivery is preferred.
- Ability to work as part of a team and communicate effectively with residents, families, and other healthcare professionals.
- A good driving record is essential, as you may be required to use your own vehicle to transport residents.
- Ability to respect individual choice in the recovery process and a commitment to recovery through the psychiatric rehabilitation process.
- Good verbal communication skills and the ability to respect the abilities and limitations of consumers.
