About the Job

We are seeking a skilled LPN Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare. As a LPN Supervisor, you will be responsible for conducting facility rounds, monitoring assignments of aides, and supervising medication assistance times.

Key Responsibilities

  • Conduct facility rounds to ensure the highest level of care for our residents.
  • Monitor assignments of aides to ensure proper work flow and efficiency.
  • Supervise medication assistance times to ensure proper recording and follow-up.
  • Intervene in resident conflicts and behavior issues.
  • Facilitate communication to ensure timely addressing of medical and mental health situations.
  • Ensure all aspects of facility incidents and accidents are completed and reported.
  • Attend to late admissions and re-admissions, including paperwork and medication changes.
  • Communicate shift activity via 24-hour report.

Requirements

  • Must be able to work as a team member.
  • Valid state LPN license.
  • Must be in good standing with State Registry.
