Clinical Plan Coordinator

The Clinical Plan Coordinator plays a vital role in assessing individual client needs and planning services to be delivered by line staff. This position requires clinical supervision of line staff, providing CSS support services to individuals with mental health diagnoses while living in a supportive housing environment.
Key Responsibilities:
- Conduct Comprehensive Rehabilitation Needs Assessments (CRNAs) and Individual Rehabilitation Plans (IRPs)
- Provide clinical supervision to line staff
- Assess individual client needs and plan services
Requirements:
- Master's Degree in a mental health-related discipline
- Current NJ Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN)
- 1+ years of experience in a supervisory role
- Ability to work effectively under pressure and manage crises
- Time and project management skills
- Ability to analyze processes and information, identify problems and trends, and develop effective solutions and strategies
- Excellent communication, interpersonal, leadership, coaching, and conflict resolution skills
- Ability to work with individuals with mental illness in a caring and professional manner
- Proficient in MS Office products and general computer literacy
- Bilingual skills are a plus
- Valid NJ Driver's License with an acceptable driving record
- Lived Experience: Collaborative Support Programs prioritizes hiring people who use their own life experience dealing with mental health/substance use issues to inspire and support others
