JAG Physical Therapy is a leading provider of outpatient orthopedic physical therapy services, with a strong commitment to delivering high-quality care to our patients. We are seeking a skilled Physical Therapist to join our team, who will be responsible for assessing patients' strengths and weaknesses, devising creative and effective treatment plans, and implementing a plan of care on a daily basis.

Key Responsibilities
  • Collect and record patient care data, including initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with the team.
  • Prepare for evaluations, including obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures.
  • Formulate diagnoses, goals, prognosis, and plans of care, and establish treatment plans based on evaluation results and best practice.
  • Collaborate with the interdisciplinary team, patients, and patient support systems to assess and re-assess the need for modifications to the plan of care and goals.
  • Identify the need for consultations and/or referrals to other healthcare professionals or services, and initiate and complete discharge plans.
  • Provide education and training to patients and/or patient support systems, including functional training, therapeutic activities, and community reintegration.
  • Responsible for treatment, plan of care, exercise prescription, and interventions for patient caseload.
  • Provide data and note writing using WEB PT EMR for all treatments provided, and oversee support staff, including PTA, ATC, and PT aides.
Requirements
  • Bachelor's or Master's Degree required, Doctorate degree preferred.
  • Licensed Physical Therapist in the state practicing in.
  • Completed state-regulated CEU requirement in the appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience/knowledge with an EMR software is preferred.
  • Excellent oral, written communication, and clinical decision-making skills.
  • Strong work ethic and dedication to patient success.
  • Basic computer skills and ability to work as a team member and individually with minimal supervision.
