Key Responsibilities
  • Monitor and evaluate medication therapy for appropriateness, ensuring optimal patient outcomes.
  • Collaborate with healthcare providers to develop and implement medication plans that meet individual patient needs.
  • Provide education and training to healthcare staff, residents, and students on medication management best practices.
  • Stay up-to-date on current federal, state, and local laws, accreditation standards, and regulatory agency requirements that apply to medication management.
  • Participate in the development of pathways, best practices, and guidelines for medication management.
Requirements
  • Doctor of Pharmacy (PharmD) degree or equivalent.
  • Current Pharmacy license in the state of practice.
  • Completion of a PGY1 Pharmacy Practice Residency or 3 years of related specialty experience.
  • Excellent communication and leadership skills.
  • Ability to work effectively in a fast-paced environment.
