Position Overview
The Registered Nurse (RN) plays a vital role in delivering exceptional patient care under the guidance of the Director of Nursing. This licensed professional applies specialized knowledge, critical thinking, and nursing expertise to support patients while adhering to the highest standards of nursing practice in a clinical environment. The RN collaborates closely with patients, families, healthcare providers, and administrative staff to ensure that care is patient-centered, cost-effective, and of superior quality. This full-time role encompasses all facets of primary care services, including operations in clinical settings, mobile health units, and community outreach initiatives.
Key Responsibilities
- Collaborate with Care Teams and clinical support staff to provide comprehensive patient care, ensuring effective communication and coordination among all team members to achieve optimal health outcomes.
- Conduct triage for patients both in-person and via telephone, managing medical inquiries according to established protocols and ambulatory care regulations. Document interventions and care plans in the electronic medical record.
- Implement and uphold clinical policies and procedures in accordance with Zufall Health Center guidelines, identifying areas for improvement and assisting in policy development.
- Oversee laboratory services in compliance with CLIA/CLIS regulations, including point-of-care testing and HIV screening.
- Act as a liaison for vaccination programs, ensuring accurate data entry and reconciliation of vaccines by support staff, and organizing necessary training sessions.
- Review laboratory and diagnostic results, flagging urgent issues for prompt attention and coordinating with care teams to inform patients.
- Provide education and counseling on primary care services, including vaccination information and women's health, in line with established medical protocols.
- Assess community health needs and coordinate responses, such as immunization drives and health education initiatives.
- Participate in health fairs and outreach events to promote community health awareness.
- Educate patients on procedures and tests, ensuring they understand the processes involved and obtain informed consent as required.
- Facilitate patient appointments with healthcare providers and specialists, coordinating care across various services and documenting interactions.
- Deliver nursing visits in accordance with established protocols, exercising sound clinical judgment and maintaining current knowledge of Zufall Health Center practices.
- Gather comprehensive patient histories and document relevant information for provider communication and evaluation.
- Administer treatments, medications, and injections as prescribed, assisting providers with medication management and refills.
- Support providers in communicating test results and additional procedures to patients, ensuring thorough documentation of follow-up plans.
- Assist patients in completing medical forms and applications for benefits, advocating for their needs with community organizations and agencies.
- Prepare analytical reports as required by grants and organizational standards.
- Identify and report any barriers to optimal patient care to the Quality Assurance and Performance Improvement Team, participating in improvement initiatives as directed.
- Maintain confidentiality of patient information in compliance with regulations and organizational policies.
- Perform additional duties as assigned, including stepping in for the Director of Nursing when necessary.
- In-depth knowledge of nursing principles and practices.
- Understanding of primary care and women's health issues in an outpatient setting.
- Strong communication skills, both verbal and written, with excellent interpersonal abilities.
- Bilingual proficiency in Spanish and English is preferred.
- Ability to work independently and manage tasks efficiently and accurately.
- Familiarity with electronic medical records and Microsoft Office applications.
- Completion of an accredited nursing program and successful passage of the National Council Licensure Examination for Registered Nurses.
- Current RN license in New Jersey and BLS certification.
- Experience in an ambulatory care or community health setting is preferred.
