Geriatric Case Manager Job Description

We are seeking a highly skilled and compassionate Geriatric Case Manager to join our team at the Northern Kentucky Area Development District. As a Geriatric Case Manager, you will play a vital role in assessing the needs of geriatric populations and providing ongoing case management services to ensure they receive the support they need to live independently.

Key Responsibilities:
  • Assess eligibility for state and federal services, including Home Delivered Meals, Personal Care, and Homemaking.
  • Provide ongoing case management services to geriatric populations, including coordination of care and support services.
  • Use a database to track client information and services provided.
  • Work offsite and participate in events to promote services and engage with the community.
  • Develop and maintain relationships with community resources and service providers.
Requirements:
  • Bachelor's degree in Social Work or a related Human Service field, or RN.
  • One year of experience in health or human services, or the educational or experiential equivalent in the field of aging or physical disabilities.
  • Valid driver's license and reliable transportation.
  • Ability to complete in-home visits throughout an eight-county region.
  • Excellent analytical, problem-solving, and decision-making skills, as well as sound judgment.
  • Ability to understand and use job-specific software and laptops/computers.
