As an Admissions Registered Nurse with Interim HealthCare of the Twin Cities, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team is dedicated to providing compassionate and personalized care to our patients, and we are seeking a skilled and dedicated RN to join our team.

Responsibilities
  • Conduct thorough assessments of patients to identify their needs and develop personalized care plans
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • Graduate of an accredited school of nursing and licensed to practice as a Registered Nurse in the state of Minnesota
  • Minimum of 12 months of experience working in an acute care or home health environment
  • Evidence of current CPR certification
  • Experience in home care, including OASIS, preferred
  • Able to travel within a geographic area serviced by the office and possess reliable transportation, a valid driver's license, and proof of auto insurance
