ROLE SUMMARY:

The Spiritual Care Counselor plays a vital role in offering spiritual guidance and support to patients and their families. This position involves direct interaction with patients or collaboration with local clergy and spiritual advisors, ensuring a holistic approach to care.

Key Responsibilities:

  • Engage in meaningful discussions about the mission and services of Pathways Home Health and Hospice.
  • Conduct thorough spiritual assessments for patients and families to tailor follow-up care.
  • Honor the dignity of patients at the end of life while respecting their spiritual and cultural beliefs.
  • Create personalized spiritual care plans that incorporate interdisciplinary insights and family preferences.
  • Provide counseling to help patients and families navigate their spiritual needs, utilizing home visits or phone consultations as appropriate.
  • Facilitate spiritual care practices, including counseling, prayer, and rituals.
  • Evaluate how cultural and spiritual beliefs influence patient care and educate the care team accordingly.
  • Lead memorial or funeral services when requested, within staffing capabilities.
  • Collaborate with the interdisciplinary group to ensure comprehensive patient and family support.
  • Participate actively in interdisciplinary meetings, sharing relevant spiritual care insights professionally.
  • Adhere to established standards of care in all settings.
  • Follow agency policies and procedures diligently.
  • Engage in quality improvement initiatives related to spiritual care practices.
  • Contribute to the development of hospice protocols and policies as needed.
  • Demonstrate proficiency in spiritual care counseling through competency assessments.
  • Document all interactions with patients and families in electronic records promptly.
  • Maintain ongoing professional development through education and peer support.
  • Implement non-pharmacologic interventions to enhance patient comfort and quality of life.
  • Facilitate communication among patients, families, and local spiritual resources.
  • Promote a culture of respect and professionalism in all interactions.

Qualifications:

  • Master of Divinity (MDiv) or equivalent graduate degree from an accredited institution.
  • Completion of clinical pastoral education (CPE) units.
  • Experience in providing spiritual care within healthcare environments.
  • Ability to work autonomously while achieving set objectives.
  • Valid driver's license and automobile insurance as per agency requirements.
  • Current CPR certification from an accredited provider.
