Job Title: Healthcare Navigator

We are seeking a dedicated and compassionate Healthcare Navigator to join our team at Native American LifeLines. As a frontline public health worker, you will build trusting relationships with community members, increasing access to healthcare.

You will work closely with staff and leadership to advance direct care initiatives, provide care coordination, and connect individuals to resources promoting health and disease prevention.

Key Responsibilities:
  • Contribute to organizational goals, adhering to policies, quality standards, and safety standards.
  • Assist individuals in understanding their health conditions and develop strategies for well-being.
  • Facilitate healthcare system navigation, identifying referrals to federal, state, and non-profit services.
  • Connect community members with routine care needs, such as prenatal care and diabetes management.
  • Advise clients on risk prevention, diagnostic screenings, and basic procedures like blood pressure screening.
  • Transport clients to scheduled appointments and enhance their ability to communicate with healthcare providers.
  • Document patient encounters and collect health data for guided initiatives.
Requirements:
  • High School Diploma or GED
  • 1 year of work or volunteer experience in a healthcare setting or dedication to Urban Indian health initiatives
Compensation and Benefits:
