About the LPN Role:
This position offers the opportunity to make a profound impact on the lives of our patients and their families by providing high-quality clinical care in a home healthcare setting. Your responsibilities will include:
- Performing patient assessments and assisting in developing individualized care plans under the supervision of a Registered Nurse (RN).
- Administering medications, treatments, and therapies as prescribed by the physician.
- Providing patient and family education on healthcare management, medication administration, and self-care techniques.
- Collaborating with the interdisciplinary team to ensure coordinated and comprehensive care delivery.
- Maintaining accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies.
- Providing emotional support and advocacy for patients and their families throughout the care continuum.
Requirements and Benefits:
To be considered for this role, you must have:
- A minimum of six months of clinical experience, preferably in a home healthcare or pediatric setting.
- Reliable transportation and willingness to travel to patient homes within the Morrisville area.
