Job SummaryThe Physical Therapist will be responsible for managing the complete process of therapeutic care for an assigned group of patients, in coordination with the physician and the patient care team. This includes providing or directing a majority of the patient's physical therapy to support patients throughout the continuum of care.
Key Responsibilities- Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Develop and implement individualized plans of care for patients.
- Collaborate with the patient care team to ensure seamless care coordination.
- Provide education and support to patients and their families on disease management and self-care.
To be successful in this role, you will need a Bachelor's Degree in Physical Therapy and a strong passion for creating positive patient interactions. You will also need excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
