The Department of Pharmaceutical Services at UCSF Health provides a comprehensive range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include drug procurement, storage, and security; safe, accurate, and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs.
Job ResponsibilitiesUnder the Department of Pharmaceutical Services, the Inpatient/Acute Care Services Per Diem Pharmacist provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed.
Key Responsibilities- Obtain medication histories and review medication profiles
- Provide drug information to patients and healthcare providers
- Collaborate with clinicians on drugs of choice and therapeutic plans
- Ensure access to medications and educate patients on proper use
- Ensure patients understand cautions, adverse reactions, and interactions related to their therapy
Under supervision, Inpatient/Acute Care Services Per Diem Pharmacists are engaged in many duties required to maintain an efficient and secure pharmacy distribution system. These duties include purchasing, storing, securing, evaluating quality of dispensing, compounding, and furnishing medications and devices in various settings.
Requirements- Must have graduated from an accredited school of pharmacy and possess a Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy
- Active license to practice Pharmacy in the State of California; must be in good standing with the California State Board of Pharmacy
- Active American Heart Association CPR Certification
