Care Coordination Specialist

Summit Medical Group is seeking a highly skilled Care Coordination Team Member to join our team. As a key member of our healthcare operations team, you will play a vital role in ensuring the smooth delivery of patient care services.
Key Responsibilities- Communication and Patient Support: Provide exceptional communication and support to patients and staff through various channels, including phone, email, and online chat.
- Appointment Scheduling and Management: Utilize our computerized scheduling system to schedule and reschedule appointments, ensuring timely and efficient management of patient appointments.
- Record Keeping and Data Management: Maintain accurate and detailed patient records using our management information systems, ensuring seamless data management and reporting.
- Referral and Authorization Management: Assist patients with completing authorizations, managing referrals, and coordinating prescription refills, ensuring timely and efficient resolution of patient issues.
- Quality Improvement and Compliance: Participate in site-level quality improvement activities, contributing to the evaluation and improvement of site performance and patient care quality.
- Training and Development: Assist in the training and orientation of new personnel, ensuring a smooth onboarding process and effective knowledge transfer.
- Education: High School Diploma required; college or vocational credit preferred.
- Experience: Clinical experience preferred; insurance company, manager care, call center, procedural, and customer service experience also preferred.
