Position Summary:
The Clinical Advanced Practice Provider, encompassing roles such as Nurse Practitioner or Physician Assistant, is responsible for delivering comprehensive care to patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) across various settings including hospitals, dialysis centers, and outpatient clinics. This role involves engaging with a diverse patient demographic, often facing resource challenges. Key responsibilities include:
- Conducting thorough assessments and managing treatment plans.
- Ordering, performing, supervising, and interpreting diagnostic and laboratory tests.
- Collaborating with MD Nephrologists to initiate and oversee treatment protocols, including prescribing medications and non-pharmacologic interventions.
- Coordinating care with patients, families, and communities, providing education and counseling.
- Offering guidance and support to nursing and medical staff to ensure adherence to best practices and evidence-based care.
Qualifications:
To excel in this role, candidates must meet the following qualifications:
- Nurse Practitioner: Completion of a graduate program from an accredited institution, national board certification, and a valid, unrestricted nursing license.
- Physician Assistant: Graduation from an accredited Physician Assistant program and successful completion of the national certification examination.
- Current Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications.
- Clear criminal background.
- Engagement in ongoing education to stay current with advancements in the field.
- Valid driver’s license and reliable transportation for travel between various clinical locations.
- Proficiency in Microsoft Office and familiarity with Electronic Health Records.
Knowledge, Skills, and Abilities:
Candidates should possess the following:
- Strong understanding of patient confidentiality and HIPAA regulations.
- Ability to work autonomously and collaboratively with healthcare teams.
- Proficient in diagnosing and managing a wide range of health issues.
- Skills in health promotion, disease prevention, and patient education.
- Excellent communication skills and the ability to interact effectively with diverse populations.
- Strong organizational skills and the ability to prioritize tasks in a dynamic environment.
