About the Role

We are seeking a skilled RN Care Manager to join our team at Billings Clinic. As a Care Manager, you will play a critical role in coordinating patient care and ensuring seamless transitions between hospital and home settings.

Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans to address patient needs
  • Collaborate with healthcare teams to ensure effective communication and coordination of care
  • Identify and address potential barriers to care, including social determinants and insurance/payer benefits
  • Provide education and support to patients and their families to promote self-management and independence
  • Monitor and evaluate patient progress, making recommendations for revisions to the care plan as needed

Requirements:

  • Four-year Bachelor of Science degree in Nursing (BSN) or Bachelor of Arts in Nursing (BAN) preferred
  • Five years of professional nursing experience in a related cardiac care area, such as critical care, ambulatory telemetry unit, or emergency services
  • Current Registered Nurse license in the state of Montana
  • National Institutes of Health Stroke Scale (NIHSS) certification required or to be obtained within first year of employment
