We are seeking a skilled and compassionate Nurse Practitioner to join our team at HOSPICE OF FRANKLIN COUNTY. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and families facing life-limiting illnesses.
Job Summary:The Nurse Practitioner will be responsible for assessing patients' physical, emotional, and spiritual needs, developing and implementing individualized care plans, and providing education and support to patients and their families. You will work collaboratively with our team of healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
Key Responsibilities:- Conduct thorough patient assessments, including medical history, physical examination, and laboratory results.
- Develop and implement individualized care plans, taking into account patients' goals, values, and preferences.
- Provide education and support to patients and their families on pain and symptom management, disease progression, and end-of-life care.
- Collaborate with the interdisciplinary team to ensure comprehensive care coordination and optimal patient outcomes.
- Document patient information accurately and timely, including progress notes, medication lists, and treatment plans.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Master's degree in nursing and current licensure as a Registered Nurse in Massachusetts.
- Advanced certification in palliative care or hospice and palliative nursing preferred.
- Minimum of three years of experience in hospice or palliative care, with a focus on pain and symptom management.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
