Job Summary

We are seeking a per diem Registered Nurse to provide nursing care according to established policies and procedures for Certified Home Health and Home Support at Seabrook Village.

Key Responsibilities
  • Develop and supervise the implementation of a resident's individualized service plan, observing the resident's condition and communicating to the Physician and family/responsible party as needed.
  • Oversee the resident's care delivered by the Home Support & Home Health team of Aides and Home Care workers and companions.
  • Partner with the initial evaluation team to assess the residents' needs and develop a service plan based on the evaluation and in conjunction with the resident, family, physician, and/or authorized practitioner.
  • Communicate any changes in the resident's condition.
  • Ensure that the resident's individualized plan is implemented and followed by the Home Support agency team of providers.
  • Deliver nursing care as outlined in the plan of care and according to acceptable nursing standards.
  • Participate in care conferences and other resident-related meetings.
  • Participate in the orientation and training of other agency nurses as well as training any Aide staff development activities.
Requirements
  • Graduate of an accredited Registered Nurse degree program.
  • Current Registered Nurse license for the state in which they practice.
  • Recent home care experience preferred.
